Immunization Reminder for Parents
Remember to add immunizations to your back to school list!
Proof of needed immunizations is required on or before the first day of school. Postcards were mailed to 8th and 12th grade parents in early March if immunizations were needed. Verify your immunization needs with your health care provider. Immunizations are available locally at South Holt Family Medicine, Mound City Family Medicine, Northwest Health Services, and the Holt County Health Department. Information is also posted on the southholtr1.com website. Exemptions are only available through the health department.
2020/2021 Booster Immunizations include:
- Entering Kindergarten - Dtap (diphtheria, tetanus, pertussis), IPV (polio), MMR (measles, mumps, rubella), Varicella (chickenpox)
- Entering 8th grade – MCV (Meningitis) and Tdap (tetanus, diphtheria, pertussis)
- Entering 12th grade – MCV(Meningitis) #2 (2 doses needed unless dose #1 was administered after age 16)
- New students are to bring current immunization records when enrolling.
